A slow cooker not getting hot enough usually shows up exactly like this: food undercooked well past the stated time, or liquid that never reaches a simmer even on high.
Before assuming the appliance is broken, it helps to know that fill level, lid seal, and even outlet voltage can each rob the crock of heat on their own. Underfilling below half full is the most common cause, since there isn't enough thermal mass to hold and distribute heat.
