Cursor Control: 4000 DPI Reality
The MX Vertical ships configured at 400 DPI by default. At 400 DPI, the cursor moves very slowly relative to physical movement — this requires larger mouse movements, which is counterproductive for an ergonomic mouse.
Immediate recommendation: open Logitech Options+ and set DPI to 1600-2400. At 1600+ DPI, cursor response matches a standard office mouse at their typical 1000-1200 DPI setting.
The higher DPI reduces physical mouse movement per cursor unit, which reduces the total wrist and arm movement during a session. This is a secondary ergonomic benefit beyond the pronation angle.
Button Layout
The MX Vertical has 4 buttons: main click, right click, scroll wheel click, and a DPI switch button on the thumb side. This is functional but minimal.
The missing element: the MX Master 3S (standard horizontal mouse) has a horizontal scroll wheel and additional thumb buttons. The MX Vertical sacrifices these for the vertical form factor.
If you use horizontal scrolling or rely on thumb buttons: this limitation is noticeable. For standard point-and-click office work: the 4-button layout is sufficient.

Would I Recommend It?
For users with forearm pronation strain: yes, unequivocally. The MX Vertical directly addresses the mechanical cause of that specific discomfort, and the results were complete symptom resolution in my case.
For users with carpal tunnel symptoms: vertical mice are less likely to help because carpal tunnel is caused by wrist flexion during mouse movement, not forearm pronation. A trackball may be more appropriate.
For users without current symptoms who want prevention: the ergonomic benefit is real but harder to quantify when there is no existing discomfort. If you already use a good desk setup and wrist rest: the incremental benefit is smaller than for users with active strain.
For users with small hands: the MX Vertical is large. At 20.3cm length and fits best for hand sizes 18cm+ (measured from wrist to middle fingertip). Small-handed users may find the grip uncomfortable.
